NHS vs Private Medical Cannabis in the UK: The Complete Comparison
Since legalisation in November 2018, UK patients have faced a stark choice: wait for an extremely rare NHS prescription, or access treatment privately. This guide explains exactly what each route offers, who qualifies, and what you can expect to pay.

What the NHS Currently Prescribes
NHS cannabis prescriptions are limited to three situations. Fewer than 1,000 NHS cannabis prescriptions are issued annually in England, compared to over 150,000 patients actively receiving private prescriptions.
1. Epidyolex for Epilepsy
Epidyolex (pharmaceutical-grade CBD) is NICE-approved for Dravet syndrome and Lennox-Gastaut syndrome — two rare, severe forms of childhood epilepsy. This is a pure CBD product with no THC, available on the NHS since 2019.
2. Sativex for MS Spasticity
Sativex (nabiximols), a 1:1 THC:CBD oromucosal spray, is NICE-approved for muscle spasticity in multiple sclerosis. Despite NICE approval in 2019, NHS availability remains patchy due to commissioning gaps between NICE guidance and NHS England implementation.
3. Nabilone for Chemotherapy Nausea
Nabilone, a synthetic THC analogue, has been available on the NHS for chemotherapy-induced nausea for several decades — predating the 2018 legalisation. It is the most consistently available NHS cannabis medicine.
Why Is the NHS Not Prescribing for Pain, Anxiety or PTSD?
The NHS follows NICE guidelines, which require robust randomised controlled trial evidence. Despite the 2018 legalisation, NICE has found insufficient RCT evidence to recommend cannabis for chronic pain, anxiety or PTSD — even as real-world evidence from private clinics accumulates rapidly.
The Project Twenty21 programme (2019–2022), coordinated by Drug Science, enrolled over 3,000 patients and generated significant real-world safety and efficacy evidence. Its successor, the ACCESS programme, continues data collection and advocates for NHS integration. Mainstream NHS prescribing for pain and PTSD is unlikely before 2027–2030.
When NHS Access Makes Sense
- Your child has Dravet or Lennox-Gastaut syndrome — Epidyolex on the NHS is the appropriate route.
- You have MS and your neurologist supports Sativex — pursue NHS first, but be prepared for commissioning delays.
- You are undergoing chemotherapy — ask your oncologist about Nabilone as part of standard anti-nausea protocols.
When Private Access Is the Right Choice
- You have chronic pain, anxiety, PTSD, insomnia, fibromyalgia or any condition outside the three NHS-approved ones.
- You need access within weeks rather than months.
- You want a tailored prescription with product and dose flexibility.
- You have already tried other treatments without adequate relief.
How to Reduce Private Prescription Costs
- Compare clinics: Mamedica charges no consultation or repeat fees — you pay only for the medicine itself.
- Use LeafMe: Compare medicine prices across all UK clinics. Savings of £30–£80/month are common on the same product.
- Choose CBD-dominant products: Lower-THC, higher-CBD products are often 40–60% cheaper and still effective for anxiety, inflammation and mild pain.
- Ask about hardship funds: Several clinics have discretionary schemes for patients in financial difficulty.
- Check research programmes: Some university hospitals offer subsidised or free access through research participation.
